Hello Educational Staff,

Your School has chosen to become a PROACTIVE SCHOOL!  A leader supporting preventative tools to  create a safer classroom.  

Well-being Safety Assessments, LLC (WSA) has been hired by your school and has conducted an online School Wellness Screening with students.   Screenings are used to identify individuals who are struggling in the areas of anger, suicide, emotional illness and drug/alcohol abuse.   It will also look at environmental safety issues and information students have about others without divulging their identities. 

WSA would also like to gather additional school background information from the educational staff and to observe all perspectives.  It is important to conduct a thorough review of your school's safety to ensure everyone's well-being.   Please read each question carefully and complete the School Background questionnaire to the best of your ability.  Wellness Screenings can save your life and the lives of others.

The information you provide is considered confidential.  WSA will only provide a summary of all the school background forms to the School Administrator.  It will not include your name, occupation or email address.  It will only include the names of students, staff and other individuals who are considered a safety concern.  This does not mean these individuals will be in trouble or have committed a crime.  The goal is to be PROACTIVE and seek intervention for any individual who is a threat to themselves or others.
